Student Registration - Student Health Records
Immunization and Health Examine Compliance
All students are required to show evidence of an Illinois physical examination upon entrance to kindergarten, grade 6, grade 9, or when entering the district from out of state(27- 8.1 School Code). All students must be in compliance in order to attend school. Students not in compliance may not attend school until meeting the requirements. Religious and medical exemptions are made with proper documentation (Board Policy 520.02). Students new to the District must participate in speech and language screening.
DENTAL EXAMS will be required for all students entering Kindergarten, 2nd grade, and 6th grade.
Vision and Hearing
Public Act 95-671, effective January 1, 2008, requires that all children enrolling in Kindergarten in a public, private or parochial school and any student enrolling for the first time in a public, private or parochial school shall have an eye examination. Each student is to present proof of having been examined by a physician licensed to practice medicine in all its branches or a licensed optometrist within the previous year before October 15 of the school year.
Much emphasis has been placed on vision and hearing problems and their effects on learning. Early detection and care of these problems is very important and will help prevent needless social, emotional and educational problems. The school nurse, a certified vision and hearing technician, will conduct vision and hearing screenings and inform the parent of the need for further evaluation. Any parent or teacher who feels that a student needs to be screened may refer then to the school nurse.
Hearing and Vision screenings will be done annually for all school age children who are in grades 1, 2, 3, and 8th; for any special education class; any referral by a teacher; and transfer students. In lieu of the screening services, a completed and signed report form indicating that the child has had an ear or vision test by a physician, audiologist, or a licensed optometrist within the previous 12 months is acceptable.
A parent or legal
guardian of a student may object to hearing and vision screenings
on religious grounds. If a religious objection is made, a
written and signed statement from the parent or legal guardian
detailing such objections must be presented to the local school
authority. General philosophical or moral reluctance to allow
hearing and vision screening will not provide a sufficient
basis for an
exception to statutory requirements.
Peru Schools District Guidelines
The School District has set certain guidelines to help you decide whether or not to send your child to school. Some diseases spread easily through a school. We try to prevent this as much as possible. We encourage frequent hand washing, proper diet and rest. Most importantly, we ask you to keep your children home when they are sick. We recognize that working parents aren't always free to stay home with sick children but, in the interest of all the children, we ask you to use good judgment and keep a sick child home.
If you have any questions for Mrs. Heuser, our school nurse, please call 223-0284. If she can not be reached immediately, leave a voice message and she will return your call as soon as possible. As always, this site is meant to provide information and is not to be used in lieu of medical care from your child's doctor.
