2007-2008 SCHOOL REGISTRATION INFORMATION

Registration Date and Times – Registration for all students of Peru Public Schools District 124 will be held on Tuesday, August 7 from 10:00 A.M. to 7:00 P.M. at Washington School.

Required Identification – ALL Parents are REQUIRED to provide TWO FORMS of evidence of CURRENT RESIDENCY within the boundaries of the school district. Proof of residency must include a driver’s license or other state-issued ID, as well as a second, recent form of identification with your name and address. This may include a recently dated rent receipt or current utility bill. Registration will not be allowed without absolute proof of residence, regardless of past attendance in the district.

Optional and Required Fees – Separate fees will be collected from Washington School students for physical education clothing and padlocks and from students who will be participating in band and chorus activities.  Students may also purchase a yearbook at this time. Detailed information will be available the day of registration. Registration fees for the 2007-2008 school year include:

                                    Developmental Preschool                  $40.00            

                                    Kindergarten through Fifth Grade       $40.00

                                    Sixth Grade through Eighth Grade      $50.00

Lunches – Hot lunch prices will be $1.90 per meal for all students.  Students at Washington must purchase a lunch card with a beginning minimum balance of $9.00 to participate.  Reduced price information based upon family income eligibility guidelines will be available during registration.

Physical and Immunization Requirements – All students are required by law to have updated immunization records.  Also, students entering kindergarten or fifth grade must provide evidence of a recent physical examination, and students entering kindergarten, second and sixth grades, as well as students previously enrolled in an out-of-state school, must provide evidence of dental exams.  All medical and dental requirements must be completed prior to admittance on the first day of student attendance on Wednesday, August 22.

Students participating in any athletic activities throughout the school year must have a completed physical examination prior to the first day of practice for their season.  For those interested, representatives from the St. Margaret’s Hospital will be present at Washington School on Tuesday, August 7 from 5:00 p.m. until 7:00 p.m. to administer examinations, which will be effective for one calendar year.  The cost for these physicals is $25.00 per student.

Birth Certificates – All students enrolling in the district for the first time are required to present a certified birth certificate, which can be obtained from the county in which the child was born.

Transportation – Students who reside more than 1 ˝ miles from the school in which they attend, or who live in an area approved as a hazardous zone by the Illinois Department of Transportation, will be provided free bus transportation to and from school. More information will be available at registration.

Opening Schedule – The first day of attendance at all schools is Wednesday, August 22 with dismissal at 12:00 Noon.  Dismissal will be at 2:00 P.M. from August 23 through August 31.  Hot lunch and transportation services will be provided.  If you would like any additional information, please contact the following schools after Tuesday, July 31:
